Kellogg’s Company Voluntary Recall of Select Packages of Frosted Mini-Wheats Bite Size Original and Mini-Wheats Unfrosted Bite Size

Kellogg Company is voluntarily recalling certain Kellogg's® Frosted Mini-Wheats® Bite Size cereal and Kellogg's®Mini-Wheats® Unfrosted Bite Size cereal. This is due to the possible presence of fragments of flexible metal mesh from a faulty manufacturing part found in certain MiniWheats® cereals produced in a single Kellogg plant between April 1 and September 21, 2012. Exposure to this product may result in minor cuts.
No injuries have been reported as of 10/07/2012. Per FDA guidance, Kellogg Company is initiating this recall as a Class ll; this requires notifying our customers to remove, quarantine and destroy any recalled product remaining in their warehouses and retail shelves.

General Mills today announced a voluntary recall of a limited number of Cascadian Farm Granola Bars containing peanuts. This action is being taken as a precaution because peanut pieces in the products may have been sourced from Sunland, Inc., a peanut supplier that recently expanded its recall of peanut ingredients.
This voluntary recall includes 6-count boxes of Cascadian Farm Peanut Butter Chip Chewy Granola Bars with the following “Better if Used By” dates printed on the top of the box: 01NOV2012 02NOV2012
No illnesses have been reported in connection with Cascadian Farm products. No other varieties or production dates of Cascadian Farm products are affected by this recall.

The Hartz Mountain Corp. is recalling some 20,000 packages of chicken jerky pet treats in the U.S. after the firm's internal tests found trace amounts of antibiotic residue, the same problem that led to earlier recalls by top suppliers.
Hartz Chicken Chews and Hartz Oinkies Pig Skin Twists Wrapped with Chicken for dogs have been pulled voluntarily from retail shelves nationwide, according to the Secaucus, N.J., firm.
Company officials said they conducted internal tests after New York state agriculture officials found evidence of antibiotic residue in treats produced by Nestle Purina PetCare Corp. and Del Monte Corp. Nestle withdrew Waggin' Train and Canyon Creek Ranch brand dog treats and Del Monte pulled its Milo's Kitchen Chicken Jerky and Chicken Grillers home-style dog treats from shelves nationwide.
In addition, two more firms recalled their treats as well, including Publix stores, which recalled its private brand Chicken Tenders Dog Chew Treats and IMS Pet Industries Inc., which withdrew its Cadet Brand Chicken Jerky Treats sold in the U.S.
Hartz officials said they found evidence of antibiotic residue in about a third of the treats they tested, but removed all the products as a precaution. They were not contacted by the New York officials, but they found the same unapproved drugs including sulfaclozine, tilmicosin, trimethoprim, enrofloxacin and sulfaquinoxaline.
Four of the antibiotics are approved in China, where most of the treats are made, and in other countries, but not in the U.S. A fifth is allowed in the U.S., but in virtually undetectable amounts in finished products.
There is no indication that the detection of antibiotic residue is related to an ongoing investigation into deaths and illnesses of dogs and cats linked to chicken jerky pet treats made in China, treat manufacturers and Food and Drug Administration officials agree. The trace antibiotic pose no health threat to pets or humans, FDA officials said.
The agency is continuing to investigate what may be behind reports of 500 deaths and more than 2,700 illnesses in dogs and cats who consumed chicken jerky pet treats made in China.

(Reuters) - Federal food inspectors have announced the recall of 1,920 pounds of sweet potato and chicken baby food made by Beech-Nut Nutrition, saying it may be contaminated with small pieces of glass.
The problem was discovered after Beech-Nut received a complaint from a consumer who found a small piece of glass in the product, the U.S. Department of Agriculture's Food Safety and Inspection Service said in a statement on Tuesday.
Beech-Nut, based in Amsterdam, N.Y., also learned of an oral injury associated with consumption of the product, it said.
"Outside of this single report, we have no indication that any other jar of our Classics Stage 2 Sweet Potato & Chicken is affected, but as a company of parents and families we are acting with an abundance of caution," Beech-Nut said in a statement.
The 4-oz. glass jars containing "Stage 2 Beech-Nut CLASSICS sweet potato & chicken" were produced on December 12, 2014.
They bear the establishment number "P-68A" inside the USDA mark of inspection, show an expiration date of "DEC 2016" and include product numbers "12395750815" through "12395750821."
Beech-Nut, which is owned by Swiss-based Hero Group, said it was encouraging customers to return the recalled product to the store where they purchased it for a refund or exchange.

From 3/25/15: Update 4/22/15:
Through a review of DNA data from listeriosis cases, federal health officials have identified two more patients who were affected by a Listeria monocytogenes outbreak linked to Blue Bell Creameries products, increasing the case count to 10.
The two cases occurred in Arizona and Oklahoma, states not previously known to be affected by the outbreak, the Centers for Disease Control and Prevention (CDC) announced today. The outbreak also includes five cases in Kansas and three in Texas. All 10 patients were hospitalized, and 3 of the Kansas patients died.
Illness-onset dates in the outbreak ranged from January 2010 through January 2015, the CDC said. Patients with illness onsets through 2014 were identified through a retrospective review of the PulseNet database for DNA fingerprints that were similar to isolates collected from Blue Bell ice cream samples.
One more isolate from a listeriosis patient is still being tested to determine if the illness is related to the outbreak, the CDC said.
Meanwhile, Blue Bell yesterday announced a recall of all of its products currently on the market, including ice cream, frozen yogurt, sherbet, and frozen snacks, because of potential Listeria contamination. Previously the company had recalled products made at its Broken Arrow, Okla., plant from Feb 12 to Mar 27, 2015.
In a statement, the company said that it now has had several positive tests for Listeria in various places and plants. "We're committed to doing the 100 percent right thing, and the best way to do that is to take all of our products off the market until we can be confident that they are all safe," Paul Kruse, Blue Bell CEO and president, said in the statement.

FDA Warning Letter: Tennessee Seafood Processor
August 10, 2015
T-Ho Foods Inc., a seafood processor in Powell, TN, was sent a July 22 warning letter from the New Orleans District Office of the U.S. Food and Drug Administration (FDA) that is very precise about rodent excreta.
In a June 1-3, 2015, inspection of the T-Ho Tennessee facility, the letter stated that FDA inspectors found:
- At least seven apparent rodent excreta pellets (aREPs) on the floor along the southeast wall of the warehouse behind the vending machine for drinks and adjacent to a pallet holding various packaging materials and plastic tubs of powdered meat bases.
- At least 13 aREPs on and under a pallet covered with a piece of cardboard along the southeast wall of the warehouse, which was holding cases of glass jar packaging materials and several paper bags of bread crumbs.
- At least three aREPs on a pallet and 35 aREPs on the floor adjacent to a pallet holding flour and bread crumbs. The pallet was located adjacent to the lift along the northwest wall of the warehouse.
- At least five aREPs on the floor in the west corner and at least six aREPs on the floor along the southwest wall of the spice room on the second floor.
- At least four aREPs on the floor in the storage area adjacent to the northeast entrance to the spice room on the second floor.
FDA noted that plenty of rodent pellets had previously been found at the T-Ho facility in both June 2013 and January 2014 in areas where food ingredients and packing materials were stored.
Among the warnings, T-Ho co-owners Thuc G. and Thoai Ho were told that they needed to take effective measures to exclude pests from these areas as good manufacturing practices require.
FDA also found that T-Ho failed to properly store equipment and remove waste and litter, nor was its building being adequately maintained as a food processing facility, particularly involving preventing drip and condensation from contaminating food.
Four of the 13 areas which FDA called to T-Ho’s attention as violations involve Hazard Analysis and Critical Control Point (HACCP) regulations and half were repeat violations, including monitoring and failure to conduct hazard analysis for each kind of fish or fish product.
FDA gave T-Ho 15 working days to respond.

Progressive Gourmet Inc. of Wilmington, MA, is recalling its 6 ounce packages of Sausage, Egg, and Cheddar Cheese on English Muffin breakfast sandwiches manufactured for Starbucks Coffee Co. out of the abundance of caution because they have the potential to be contaminated with Listeria monocytogenes. Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.
The product comes in a 6 ounce, clear plastic labeled package marked with Best Before: 07-AUG-2016 on the top. Note, no other production runs of this product are impacted by this recall.
These potentially impacted sandwiches were possibly sold to consumers by select Starbucks stores in Arkansas, Texas and Oklahoma Starbucks Stores on 3/3/16 and 3/4/16. The sandwiches may be heated at the store level for immediate consumption.
This recall is limited to impacted 250 stores in Arkansas, Texas and Oklahoma only. (The recalled sandwiches were distributed to warehouses in Maryland, North Carolina, Texas and Louisiana).
As soon as Starbucks was informed of this potential issue, the impacted product was removed from the 250 stores that potentially received it. The scope of this issue is limited to these stores.
The potential for contamination was noted after routine testing by the company revealed the presence of Listeria monocytogenes on a contact surface within the production facility.
No illnesses have been reported to date in connection with this product.
Progressive Gourmet is working closely with FDA and the Massachusetts Department of Health and continues to investigate the source of the problem.
Consumers who have purchased 6 ounce packages of the sandwich are urged to return them to the place of purchase for a full refund. Consumers with questions may contact the company at 1-800-224-7630 (9:00 am-5:00 pm Monday through Friday EST).

Tri-Union Seafoods LLC is voluntarily recalling a 2,745 cases (equal to 107,280 cans) of Chicken of the Sea brand 5 oz. canned chunk light tuna in oil and 5 oz. canned chunk light tuna in water.
This recall has been initiated because the product may have been undercooked due to an equipment malfunction, which was uncovered during a routine inspection. These deviations were part of the commercial sterilization process and could result in contamination by spoilage organisms or pathogens, which could lead to life-threatening illness if consumed. There have been no reported illnesses to date connected with the affected products, and Tri-Union Seafoods LLC is issuing this voluntary recall to ensure the highest level of safety and quality.
The UPC code, Best By date and lot codes are used to determine the products involved. The UPC code (also known as the bar code) is found on the label of the product. Bumble Bee Foods, LLC Issues Voluntary Recall on 3 Production Codes of Canned Chunk Light Tuna Due to Possible Health Risk

SAN DIEGO – Bumble Bee Foods, LLC announced today that it is voluntarily recalling 3 specific UPC codes of canned Chunk Light tuna due to process deviations that occurred in a co-pack facility not owned or operated by Bumble Bee. These deviations were part of the commercial sterilization process and could result in contamination by spoilage organisms or pathogens, which could lead to life-threatening illness if consumed. It is important to note that there have been no reports of illness associated with these products to date. No other production codes or products are affected by this recall.
There are a total of 31,579 cases that are included in the recall which were produced in February 2016 and distributed nationally. The products subject to this recall are marked with a can code that starts with a "T" (example: TOA2BSCAFB) and have the following "best by" dates:

Awareness Voluntary Recalls Boost Tea Due To Possible Salmonella Contamination

Awareness Corp. of Mesa, AZ, is recalling its 7.4 ounce container of Boost Tea because it may be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonellaoften experience fever, diarrhea, nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream area and producing more severe illnesses such as arterial infections, endocarditis and arthritis.
NO ILLNESSES HAVE BEEN REPORTED IN CONNECTION WITH THIS VOLUNTARY RECALL.
The possibility of Salmonella contamination was noted after what we have been informed was a routine testing by a third party customer of FutureCeuticals, a large manufacturer and supplier of organic ingredients in North America. We have been informed by a reliable source that organic spinach powder produced by FutureCeuticals tested positive for Salmonella in another company’s product, not in Boost Tea. Boost Tea also contains this organic spinach powder ingredient obtained from supplier FutureCeuticals.
Upon notification by the FDA of FutureCeuticals Spinach powder issues, Awareness conducted its own investigation and had independent lab analysis performed by two separate analytical laboratories which confirmed a complete absence of Salmonella in its Boost Tea product. Based on Awareness’ utmost concern about product safety, it is conducting this voluntary recall as a preventative measure. To prevent a recurrence of this concern, Awareness has removed organic Spinach powder from Boost Tea and expects new Boost Tea to be available in the next few weeks.
The voluntarily recalled Boost Tea was distributed nationwide by distributors from Awareness. The product’s white plastic container is marked with Lot #022615 on the bottom panel of the container and with an expiration date of 10/17 stamped on the container. It was first sold to distributors on or about December 15, 2015.

H-E-B Issues Voluntary Recall: Select 5oz Hill Country Fare Chunk Light Tuna in Oil Recalled for Potential Health Risk

San Antonio, Texas – H-E-B is voluntarily recalling 224 cases (10,752 cans) of Hill Country Fare brand 5 oz. Chunk Light Tuna in Oil.
This recall has been initiated because the product, produced at a co-packer, may have been undercooked due to an equipment malfunction, which was uncovered during a routine inspection. These deviations were part of the commercial sterilization process and could result in contamination by spoilage organisms or pathogens, which could lead to life-threatening illness if consumed. There have been no reported illnesses to date, H-E-B is issuing this voluntary recall in abundance of caution to ensure the highest level of food safety and quality.

FDA report shows Dole officials knew about Listeria in facility

Company officials knew a Dole salad plant was contaminated with Listeria for a year and a half before they shut it down — then they only took action after the U.S. and Canadian governments traced a deadly outbreak to the facility....

Frontera Foods Issues Allergy Alert for Undeclared Soy Allergen in Original Taco Skillet Sauce with a "Best By" Date of "13 Apr 17"
Frontera Foods of Chicago, Illinois is recalling a total of 720 (8 oz.) packages of its Frontera Texas Original Taco Skillet Sauce because it contains undeclared soy. People who have an allergy or severe sensitivity to soy may run the risk of a serious or life-threatening allergic reaction if they consume this product.
The recalled Frontera Texas Original Taco Skillet Sauce packages were only distributed to District Columbia and the following 11 states; Connecticut, Massachusetts, Maryland, Maine, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, Virginia and Vermont. The recalled product went to the specific retail stores on the attached list.

The initial report (email) that I received was the first one listed below. After that, I received quite quickly all the others. They all stem from SunOpta, and Giant Eagle, I believe.

(NYSE: KR) said today it is recalling Kroger Sunflower Salad Toppers, Kroger Sunflower Kernels, and Kroger Cranberry Delight Trail Mix because a supplier indicated sunflower kernels may be contaminated with Listeria monocytogenes, an organism which could result in severe illness to those individuals who may consume these products. No customer illnesses have been reported to date.

Brown & Haley of Tacoma, Washington is taking the precautionary measure of voluntarily recalling certain lots of Mountain Thins "Trail Mix" flavor that may be impacted by sunflower seeds contaminated with Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, listeria infection can cause miscarriages and stillbirths among pregnant women.

Gold Medal, Gold Medal Wondra, and Signature Kitchens Flour Recalled Due to Possible E. coli O121 Contamination

MINNEAPOLIS – General Mills is collaborating with health officials to investigate an ongoing, multistate outbreak of E. coli O121 that may be potentially linked to Gold Medal flour, Wondra flour, and Signature Kitchens flour (sold in Safeway, Albertsons, Jewel, Shaws, Vons, United, Randalls, and Acme). Out of an abundance of caution, a voluntary recall is being made. To date, E. coli O121 has not been found in any General Mills flour products or in the flour manufacturing facility and the company has not been contacted directly by any consumer reporting confirmed illnesses related to these products.
State and federal authorities have been researching 38 occurrences of illnesses across 20 states related to a specific type of E. coli (E. coli O121), between December 21, 2015, and May 3, 2016. While attempting to track the cause of the illness, CDC found that approximately half of the individuals reported making something homemade with flour at some point prior to becoming ill. Some reported using a General Mills brand of flour.
Based on the information that has been shared with General Mills, some of the ill consumers may have also consumed raw dough or batter. Consumers are reminded to not consume any raw products made with flour. Flour is an ingredient that comes from milling wheat, something grown outdoors that carries with it risks of bacteria which are rendered harmless by baking, frying or boiling. Consumers are reminded to wash their hands, work surfaces, and utensils thoroughly after contact with raw dough products or flour, and to never eat raw dough or batter.

General Mills has recalled an estimated 10 million pounds of flour over fears that they contain the E. coli virus. Over 38 people have been stricken with the virus in over 20 states, and General Mills has linked it to their flour.
This E. coli outbreak is caused by the strain O121, which causes cramps, bloody diarrhea, vomiting and can potentially lead to a life-threatening situation. The Centers for Disease Control noted that most people who became ill with this particular strain recall baking something homemade prior to becoming ill.
Several in this group noted that they have used the General Mills brand of flour. It was also noted that many of those who presented symptoms of E. coli had also eaten raw dough or batter while making their homemade treats, which General Mills specifically advises against.
General Mills also notes that older and very young individuals, and those with compromised immune systems are most at risk for developing E. coli. The particular populations should take special precautions not to consume raw dough or batter and to wash their hands thoroughly after touching flour.
The company stated:
“While have not found any presence of E. coli O121 in any General Mills flour products or in the flour manufacturing facility, and we have not been contacted directly by any consumer reporting confirmed illnesses related to these products, we want to take an active part in helping to prevent food borne illnesses.”General Mills has urged anyone who thinks they may have E. coli to contact their doctor. If they are diagnosed, they are asked to report to their local health authority.

Individuals engaging in cooking projects should be aware that baking or cooking items with flour in them will automatically destroy any bacteria. Consumers should also take care to wash off any utensils or cooking items that come into contact with flour thoroughly after use.

Bolthouse Farms is voluntarily recalling a selection of protein drinks due to possible spoilage that may cause the beverages to appear lumpy, taste unpleasant and have an off odor.
These products should not be consumed. The issue was identified after the company received consumer complaints, including reports of illness. The cause of this issue is currently under investigation.

More sick; General Mills recalls more flour traced to outbreak

As the E. coli outbreak traced to General Mills flour continues to grow the giant food company has again expanded its recall of flour and renewed its warnings to consumers to not eat raw flour.
Four more people are confirmed with E. coli infections, bringing the count to 46 people across 21 states, according to a Monday afternoon update from the Centers for Disease Control and Prevention.
No one has died, but 13 victims have required hospitalization and one has developed the serious complication hemolytic uremic syndrome (HUS), a type of kidney failure that is often fatal. The first 42 outbreak victims were infected with E. coli O121.
