    After Ebola, a Blueprint Emerges to Jump-Start R&D

    What’s newsworthy, however, is the current pace of Ebola research and development. In the last two months, an Ebola candidate vaccine has entered final stage testing in humans in Guinea and Sierra Leone. While it typically takes between five to ten years to develop and fully test a vaccine, because of the urgency provided by the Ebola epidemic, the pace of science was drastically accelerated.

    On the other side of the laboratory, new diagnostics now allow for a much quicker diagnosis of the virus. Three new drugs are being tested to assess whether they are able to cure Ebola patients from the disease. ***

    The Ebola epidemic in West Africa has been called a wake-up call for so many reasons. With Ebola, once the world understood the threat, the actions taken not only halted the disease, but also left us better prepared to fend off the next outbreak.

    From where I sit in Geneva, the response by the global health and R&D communities awakens hope. There are many diseases with the potential to trigger chaos and human suffering, but we have learned how to jumpstart research and development to address epidemics as they occur and stem the tide of suffering from these diseases. We have also learned that we must take some critical research steps proactively before another wake-up call.

    We are now in a position to design a framework for how to prepare for and respond in the most challenging public health crises. That framework needs fine tuning, and the support of the international community so that next time we are prepared to move forward more quickly, and halt the epidemic before it becomes the next dire crisis.

    Since the week ending 10 May, when a 10-month low of 9 cases of Ebola virus disease (EVD) were reported from 2 prefectures of Guinea and 1 district of Sierra Leone, both the intensity and geographical area of EVD transmission have increased. In the week ending 31 May, a total of 25 confirmed cases were reported from 4 prefectures of Guinea and 3 districts of Sierra Leone. Several cases in both Guinea and Sierra Leone arose from unknown sources of infection in areas that have not reported confirmed cases for several weeks, indicating that chains of transmission continue to go undetected. Rigorous contact tracing, active case finding, and infection prevention and control must be maintained at current intensive levels in order to uncover and break every chain of transmission. However, the onset of the rainy season will make field operations more difficult from now onwards.

    Germany's Angela Merkel is leading the G-7 charge to seriously reform the World Health Organization.
    The G-7 Summit in Germany convening June 7 to June 8 will focus on the epidemic threat agenda, framed both as biosecurity and health system strengthening. Detailed G-7 draft documents are not yet available, so the precise outlines of Germany’s plan aren’t visible. But German Chancellor Angela Merkel left clues in the speech she delivered at the World Health Assembly (WHA68 ) in Geneva in May. She said: “We need some kind of global disaster response plan. And the World Health Organization must play a key part in this. But first of all we have to ask ourselves what we expect from the World Health Organization, what we think it should achieve and what its member states have to provide.”

    MSF’s Joanne Liu says the G7 summit failed to produce any proposals for a rapid response to major epidemics in distant countries

    Aid donors missed a golden opportunity at the G7 summit in Germany to lay out measures for a far more vigorous response to epidemics such as the Ebola outbreak, according to Joanne Liu, the head of Médecins Sans Frontières (MSF).

    “There were strong opening remarks from Angela Merkel at the G7, saying we have failed on Ebola, we take full responsibility, but at the end, the recommendations were all watered down,” said Liu.

    The UN’s financial tracking system shows that the scale of giving among G7 states varied greatly. Italy gave 0.3% of the total Ebola budget, while the US contributed about 45% . The UK gave roughly 10%.

    But, frustratingly, the $1.7bn of US funding did not reap the rewards it could have, Liu noted.

    “Unlike the Cubans, the US were not ready to expose their people. So even though they opened 11 centres with 150 beds each in Liberia, they treated just 28 people, because they deployed late, and refused to staff the centres.

    Africa is poised for another disease outbreak and may not do any better than last time.

    ...New cases of Ebola have been reported.

    “While the virus is spreading more slowly than at the peak last year and Liberia has defeated the outbreak,” according to one news source, “cases have risen sharply in the past two weeks in Guinea and Sierra Leone.” There are also concerns that the region may not be much better prepared to stem an outbreak than it was last year.
